As the hirer, appointee or nominee, read this statement and confirm that you understand and accept it.

In order to add  to this policy we have asked you to provide details of their driving record.

Any incorrect information may result in removal of the driver without further notice and refusal to provide cover for any accidents or losses for this driver.

You must also inform us of any future changes that may affect your policy cover (for example, motoring convictions, accidents or if you're advised not to drive due to a medical condition).
