Driver declaration

The DVLA share information about your licence so that we can check if you are eligible to be a driver on the Scheme. Please confirm that you agree to this.

I, , acknowledge that Motability Operations Limited will ask DVLA for my driver record information, as and when they require, for the purpose of verifying my driver licence details and eligibility to be a named driver on the Motability Scheme for insurance purposes.

I understand that DVLA will disclose to Motability Operations Limited all relevant information held in the computerised register of drivers maintained by DVLA. This includes personal details, driving entitlements, valid endorsements and disqualifications (if relevant). This declaration will expire when I cease to drive in connection with the Motability Scheme or three years from the date of this acknowledgement.
