Before you start

To add the driver to the policy, they will need to be with you to fill in the form.

They will need to provide the following information:

• personal details
• licence details, including licence number and date they passed their test
• claims history for the past 5 years
• details of any motoring convictions, penalty points, bans or disqualifications in the last 5 years
• any restrictions on their licence

Their cover will begin on successful completion of this form.
